Enrollment

Qualified applicants can proceed with enrollment by submitting the following documents onsite:

  • Duly accomplished Basic Education Enrollment Form

  • Certified True Copy of School Form 9 (formerly Form 138-E), covering:

    • → Grade 6 (First to Fourth Quarter)
